Key Responsibilities

As a Community Care Partner, your primary tasks will involve:

• The execution of community-based population health activities in adherence to performance goals.
• Collaborating with community organizations and healthcare providers to enhance awareness and participation in preventive health campaigns.
• Developing relationships with community organizations to penetrate new markets and expand outreach efforts.
• Coordinating and tracking outreach data, organizing educational and screening events, and managing volunteer teams.
• Ensuring compliance with quality regulations and minimizing business risks.
• Presenting educational sessions on health/safety and conducting outreach workshops to improve member knowledge.
• Making member referrals for coordinated care services in collaboration with community health providers.
• Managing travel within the Memphis/West Tennessee area, sometimes at short notice and up to 50% of work time.
• Occasionally lifting weights between 30 to 40 lbs.

Qualifications

Education

• A Bachelor's Degree in a health-related field is required, including disciplines such as Nursing, Dietetics, Exercise Science, Counseling, Social Work, or Health Education.

Experience

• A minimum of three years of experience in project management, account management, or related fields.
• Must have three years of experience in planning, developing, and implementing comprehensive prevention and community outreach programs.

Skills and Certifications

• Proficiency in Microsoft Office Applications (Outlook, Word, Excel, Access, and PowerPoint).
• Strong oral and written communication skills.
• Excellent interpersonal and organizational skills.
• Proven decision-making, problem-solving, and analytical thinking skills.
• Strong presentation capabilities and the ability to work independently under general supervision as well as collaboratively within a team.
• A valid Driver’s License is necessary for the role.
